LiveScan Fingerprinting and FD-258 cards

UTPD is proud to offer comprehensive fingerprinting services, including Livescan Fingerprinting Submissions, Driver Privilege Cards (with photo), traditional wet-ink fingerprints on forms, and FD-258 Printed Fingerprint Cards.

We serve both the public and the Utah Tech community, providing efficient and accurate fingerprinting services. Our Livescan Fingerprints are electronically submitted to the Bureau of Criminal Identification (BCI), ensuring quick and secure processing. If needed, we can also provide your fingerprints on the standard FD-258 cards, which are readily available in our office.

If you have a Social Security number, you will be asked for the number at your appointment.

WET-INK FINGERPRINTS ON FORMS
For specific applications, such as the Application for Criminal History Record or the Application for Certificate of Eligibility, we offer wet-ink fingerprinting at no charge. These forms are available in our office and can also be accessed online at https://bci.utah.gov/criminal-history-records/.

RIGHT OF ACCESS AGENCY
As a Right of Access agency, our department is equipped to conduct Utah Criminal History Checks for the public. Feel free to contact our office to see how we can assist you. For more details about Utah Criminal History, simply scroll down the page.

 

APPOINTMENTS are available Monday through Friday, by appointment only. (Closed major holidays.)

DIRECTIONS
From St. George Blvd: Head west and turn left onto 700 East (University Ave).
On 700 East: Drive south until you reach 300 South (Trailblazer Blvd).
On 300 South: Turn left, then take the first right into Parking Lot M.
In Parking Lot M: Drive south towards the Anthony Burns offices. The University Police Department is located in the northeast corner of the Anthony Burns offices, near the line of palm trees, just south of the Holland Library.

PARKING
For fingerprint services, if available, park in a stall marked “University Police.”
If those stalls are occupied, please use a Visitor parking stall, which are marked in green.

IDENTIFICATION
One of the following valid government photo identifications (ID) is required:
Alien Registration Card
Driver’s License
Military ID Card
Passport (if your passport is expired you must bring in a photo ID along with the passport.)
State Identification Card

REQUIREMENTS & FEES

DIRECT SUBMISSION TO BCI (BUREAU OF CRIMINAL IDENTIFICATION):
*Requirements: Livescan authorization form (you provide form), valid passport or photo ID and payment
*Real Estate Renewals – Choose this option. Go to: https://db.realestate.utah.gov/rapback/ to obtain form and bring to appointment
*POST Academy Applicants – Choose this option (no form required.)
*Utah Tech Students involved in the Americorp or EMS program must bring a Livescan Authorization Form. (Billing will be handled.)
*Utah Tech Students or Employees working in campus childcare must bring a Livescan Authorization Form. (Billing will be handled.)

FINGERPRINT CARDS (not for Driver Privilege ID) – INCLUDES TWO FD-258 CARDS:

DRIVER PRIVILEGE (FOR DMV) – INCLUDES TWO FD-258 CARDS AND ONE PHOTO, SEALED IN AN ENVELOPE, WHICH YOU WILL TAKE TO THE DMV:
*Requirements: Valid passport or photo ID and payment

TSA/FLIGHT SCHOOL – INCLUDES FOUR FD-258 CARDS.  Bring a self-paid trackable envelope (Fedex or USPS) and the email you received giving you permission to get the prints done. If you do not have all of these items, you will need to reschedule your appointment:
*Requirements: Printed email giving permission, valid passport or photo ID and payment
Pay $40 for four FD-258 fingerprint cards

If you need an additional card than what is included, pay $2.00 per card.
Pay $2 per card

PAYMENT RECEIPT
Payments must be made prior to or at the time you come to our office. You must provide a receipt of online payment by a print or show proof from your phone. (Online payment links above.) If you are unable to make an online payment, we accept cash payment. Exact change is preferred. You may also pay with check at the Cashier’s Office located on the 1st Floor of the Holland Building (HCC) on campus prior to your appointment.

Criminal History Check

RIGHT OF ACCESS AGENCY
As a Right of Access agency, the Utah Tech Police Department (UTPD) is authorized to conduct Utah Criminal History Checks for the public. If you need assistance, please feel free to contact our office.

REQUESTING A UTAH CRIMINAL HISTORY
Appointments are available during regular office hours, Monday through Friday, 9:00 AM to 5:00 PM. To schedule an appointment, please call 435-652-7515 during office hours or email policeservices@utahtech.edu.

FEES
Volunteers & Citizens: $15.00 for a copy of their own Utah Criminal History.
Municipalities: $10.00 for municipal services.

You may print the Application for Criminal History from our website (link below), but it must be signed in the presence of a UTPD Police Records employee—No Exceptions. Forms are also available in our office.

Notary Services

Utah Tech Police Department (UTPD) provides Notary Services for a $5.00 fee per document. To make an appointment, call 435-652-7515 or email policeservices@utahtech.edu.

Notary services are by appointment only.

Upon arriving for your appointment, you may pay with cash or card. (Online payment link below.)

Please bring a government issued photo ID, documents requiring notarization, and show payment confirmation (either email confirmation or paper receipt).

Resources for Students, Faculty, and Staff

S.A.F.E. (Self-Defense Awareness & Focused Empowerment)

Utah Tech Police Department (UTPD) offers a free course covering self-protection and self-defense. The class is offered to Utah Tech University faculty, staff, and students. The purpose and goal of the class focuses on awareness, assault prevention, risk reduction, strategies and avoidance in conjunction with defensive training tactics.

The S.A.F.E. class will help educate and train specifically on how to defend yourself physically from assaults. The classes are taught by Utah Tech University Police Officers and Staff.

The S.A.F.E. class is a 4.5-hour course broken down into two classes. Classes are limited to 20 participants and are filled on a first-come, first-served basis. You will be required to sign a liability waiver in order to participate and you must attend both classes.
