LiveScan Fingerprinting and FD-258 cards
UTPD is proud to offer comprehensive fingerprinting services, including Livescan Fingerprinting Submissions, Driver Privilege Cards (with photo), traditional wet-ink fingerprints on forms, and FD-258 Printed Fingerprint Cards.
We serve both the public and the Utah Tech community, providing efficient and accurate fingerprinting services. Our Livescan Fingerprints are electronically submitted to the Bureau of Criminal Identification (BCI), ensuring quick and secure processing. If needed, we can also provide your fingerprints on the standard FD-258 cards, which are readily available in our office.
WET-INK FINGERPRINTS ON FORMS
For specific applications, such as the Application for Criminal History Record or the Application for Certificate of Eligibility, we offer wet-ink fingerprinting at no charge. These forms are available in our office and can also be accessed online at https://bci.utah.gov/criminal-history-records/.
RIGHT OF ACCESS AGENCY
As a designated Right of Access agency, our department is authorized to conduct Utah Criminal History checks for the public. Please contact our office to learn how we can assist you.

APPOINTMENTS are available Monday through Friday, by appointment only. (Closed major holidays.)

IDENTIFICATION
One of the following valid government photo identifications (ID) is required.
If the ID is expired, you must bring in a second form of ID:
Driver’s License
Military ID Card
Passport
State Identification Card
1.DIRECT SUBMISSION TO BCI (UTAH BUREAU OF CRIMINAL IDENTIFICATION)
What You Need:
Livescan Authorization Form (you bring this)
Valid photo ID or passport (Any ID invalid, bring another form of ID)
$20 payment
How to Get Your Livescan Authorization Form:
Issued by the Utah Department of Health & Human Services (DHHS) – bring it to your appointment.
Utah Real Estate Renewals:
Visit https://db.realestate.utah.gov/rapback/ to get your form.
Tow Truck Drivers and Utah Healthcare Workers:
Bring your authorization form.
Utah Tech POST Academy Applicants:
No form needed – just choose this option.
Utah Tech Students/Staff (AmeriCorps, EMS, or Childcare programs):
Bring your authorization form. (Billing is handled separately.)
2. FINGERPRINT CARDS (not for Driver Privilege ID) – INCLUDES TWO FD-258 CARDS
What You Need:
Valid photo ID or passport (Any ID invalid, bring another form of ID)
$25 payment (includes two FD-258 fingerprint cards)
Choose this option if:
You are a new Utah Real Estate applicant, new licensing (i.e. nursing), completing paperwork for adoption, going on a church mission, or required to submit prints for court.
You need to mail or deliver fingerprint cards to a third party.
3.DRIVER PRIVILEGE (FOR DMV) – INCLUDES TWO FD-258 CARDS AND ONE PHOTO, SEALED IN AN ENVELOPE, WHICH YOU WILL TAKE TO THE DMV
What You Need:
Valid photo ID or passport (Any ID invalid, bring another form of ID)
$35 payment (Covers two FD-258 fingerprint cards and one photo)
4.TSA/FLIGHT SCHOOL – INCLUDES FOUR FD-258 CARDS. Bring a self-paid trackable envelope (Fedex or USPS) and the email you received giving you permission to get the prints done. If you do not have all of these items, you will need to reschedule your appointment
What You Need:
Printed email giving permission and valid passport or photo ID
$40 payment (Covers four FD-258 fingerprint cards)
5.If you need an additional card than what is included, pay $2.00 per card.
6.If you require wet-ink on a form, like the Application for Criminal History, that’s available at no charge and we have the forms in our office.
PAYMENT RECEIPT
Payments can be made prior to or at the time you come to our office. If you are unable to make an online payment, we accept cash payment. Exact change preferred. You may also pay with check at the Cashier’s Office located on the 1st Floor of the Holland Building (HCC) on campus prior to your appointment.
PARKING
The parking spaces marked as “University Police” can be utilized for fingerprint appointments. If not available, park in a green marked visitor stall.

Criminal History Check
RIGHT OF ACCESS AGENCY
As a Right of Access agency, the Utah Tech Police Department (UTPD) is authorized to conduct Utah Criminal History Checks for the public. If you need assistance, please feel free to contact our office.
REQUESTING A UTAH CRIMINAL HISTORY
Appointments are available during regular office hours, Monday through Friday, 9:00 AM to 5:00 PM. To schedule an appointment, use the fingerprint calendar that’s above and in the “Comments” section specify Utah Criminal History. Or call 435-652-7515 during office hours or email policeservices@utahtech.edu.

FORMS
You may print the Application for Criminal History from our website, but it must be signed in the presence of a UTPD Police Records employee—No Exceptions. Forms are also available in our office.

Pay $15 for Volunteers & Citizens in Utah seeking their own Utah Criminal History
Pay $10 for Utah Criminal History – Municipal Services (City/County/State-Utah)

Notary Services
Utah Tech Police Department (UTPD) provides Notary Services for the public and the Utah Tech community.

REQUIREMENTS
Please bring a government-issued photo ID and the documents that require notarization.
Do NOT sign or date the documents before your arrival, as the notary MUST witness you signing them.

Resources for Students, Faculty, and Staff
S.A.F.E. (Self-Defense Awareness & Focused Empowerment)
Utah Tech Police Department (UTPD) offers a free course covering self-protection and self-defense. The class is offered to Utah Tech University faculty, staff, and students. The purpose and goal of the class focuses on awareness, assault prevention, risk reduction, strategies and avoidance in conjunction with defensive training tactics.
The S.A.F.E. class will help educate and train specifically on how to defend yourself physically from assaults. The classes are taught by Utah Tech University Police Officers and Staff.
The S.A.F.E. class is a 4.5-hour course broken down into two classes. Classes are limited to 20 participants and are filled on a first-come, first-served basis. You will be required to sign a liability waiver in order to participate and you must attend both classes.
